Care Management Plus Tools


The Care Management Plus project offers several useful tools for improving the effectiveness and efficiency of care management in the clinical setting.

1. The Care Management Tracking software: This database allows the provider to manage patient interactions more efficiently and in more detail than most electronic medical records. Please click the link at left for more information.

2. The Patient Summary Sheet: The Patient Worksheet is a one or two page printed report that contains a structured summary of a patient's longitudinal health information from the electronic medical record. Examples and more discussion of the uses for this are available under the link at left.
